8 June 1968 Lt. Smith crashed YZ-20 / 144645 due to suspected Tail Rotor Failure, the Aircraft caught fire and burned, the aircraft was a total loss. . Lt. Smith received 1st & 2nd degree burns of the face and arms, the gunner, LCpl Avery received a broken leg and 2nd degree burs to one arm and one leg.
